Most are unaware of the awareness and complications that vitamin K-rich foods bring to their current health status. Clients are also unaware of the need to adjust their warfarin dosage and monitor the international normalization ratio (INR) level. Although nurses provide prompt verbal instructions, such as informing patients to report immediately to the provider if they observe bruising on the skin, excessive bleeding from the gums, or blood in the urine, patients do not fully understand the danger of taking anticoagulant medications.
